Instructions:
(1) Complete Application except for SCHOLARSHIP NAME, List of criteria
and Description of how you meet each criterion.
(2) Make enough copies of this Application for each scholarship
for which you are applying.
(3) Fill in the SCHOLARSHIP NAME. One Application is required for
each scholarship for which you are applying.

(4) In the column so indicated below, list each criterion for the
scholarship (as set forth in the attached pages listing the scholarship
criteria or from the Web site listed above) and then, in the other
column, below, opposite each criterion, describe in a brief statement
how you meet/satisfy that criterion.
(5) No letters of recommendation are required unless otherwise stated
in scholarship criteria.
(6) Completed packets (stapled in the upper left hand corner) should
be returned to your high school guidance director. One completed
application is required for each scholarship for which you are applying.
Ask the guidance director to attach your high school transcript with the first quarter of senior year and including the ACT results, to each application and mail to this
office on or before Feb. 15 . A completed packet includes
this Application, your Transcript including the ACT results and
letters of recommendation where requested. Incomplete applications
or applications not meeting the criteria will not be considered.

be awarded this scholarship.
The above information is correct and complete to the best of my
knowledge. I understand, agree, and consent to the use by the selection
committee of the information contained in and submitted with this
application. I am aware that the letters of recommendation (when
required) may be verified. I understand any award made is contingent
upon my enrollment at UW-L as a full time student (unless otherwise
stated) at the time the award is paid. I hereby state and affirm
that I do meet all the criteria for the scholarship for which I
am applying. I understand that awards from this application are
being made for the 2006-2007 academic year payable in two equal
installments in September and January. I further understand the Foundation and university reserves the right to publish names of scholarship recipients.
