I am aware that the Letters of Recommendation may be verified. I understand any award made to me is contingent upon my enrollment at UW-L as a student in good standing at the time the award is paid. I do meet all the criteria for the scholarship. Awards from this application are made for the next academic year, payable in two equal installments. I further understand the Foundation reserves the right to publish names of scholarship recipients.

Students should complete this application and return the completed packet to the UW-L Foundation Office no later than March 1. Scholarship applications will be verified by the OCW Scholarship Committee and presentations made to the recipient whose qualifications best fit the stipulations of the scholarship. Scholarship files (including the letters of recommendation) and any other records are confidential and not available for inspection by applicants or any others without the written consent of the UW-L Foundation and the OCW Scholarship Committee. Incomplete applications will not be accepted. Deadline is March 1st.
